WORLD QUALIFIER TEAM CERTIFICATION NEEDS TO BE COMPLETED AND EMAILED BEFORE THE TOURNAMENT! 

This form needs to be completed with a signature from all eligible team members (Fill out the back of the form). You WILL NOT be allowed to play in the State tournament if this form is not completed.

IF YOU FEEL A PLAYER on your team should be ranked higher this is your last chance to raise them.

Please do not request that a players skill level be lowered on this form.

MAKE SURE TO HAVE THIS COMPLETED FORM IN THE OFFICE NO LATER THAN NOON ON THURSDAY, JUNE 12TH.

If not received by noon, your team will not be allowed to pick up your scoresheet and play at the tournament until the form is received.

Each eligible member of your team will need to provide a current address, signature and SOCIAL SECURITY NUMBER.

I ask that you have this form completed and hold on to it until your team wins. If your team loses, you can shred the form.

Players will need to provide their social security number in order to receive their flight reimbursements.

If you have someone who won't be able to make it to the tournament; please make sure to have them fill the sheet out ahead of time.

Each player who receives over $600 in prizes will receive a 1099. We include the tournament entry fee*, hotel cost* and flight reimbursement.

Hotel Room Reservation Sheet

Please provide us with the names of the 3 players you would like your rooms under.

I do not need to know who is staying in whose room; I just need the name of 3 players to put the rooms under.

Those 3 players will need to be able to provide a credit card (or a cash deposit)at check in for any incidentals.

We will provide each team with 3 rooms (double beds) to check in the day before the event and check out the final event day. Those dates will be provided in booklet team gets when they check in at the World Qualifying tournament.
